1. The administrator of your personal data is the Wrocław University of Health and Sport
Sciences, al. Ignacego Jana Paderewskiego 35, 51-612 Wrocław;
2. The accuracy of personal data processing is supervised by the Data Protection Officer,
who can be contacted via e-mail at iod@awf.wroc.pl;
3. Your personal data will be processed for the purpose of the enrolment procedure and
documenting the course of your studies and will be used for statutory, archival and statistical
purposes;
4. The basis for processing of personal data of candidates for studies is the Act of 20
July 2018. The Law on higher education and science (Dz.U.2023.742 t.j.);
5. Providing the data is voluntary, but necessary for the purposes for which it was collected;
6. The data will not be shared with any external entities, except in cases provided for by the
law;
7. The data will be stored for the time necessary to fulfil the purposes set out above;
8. You have the right to access your data and, subjected to the limitations of the provisions
of law, the right to rectification, erasure, restriction of processing, the right to data portability,
the right to object, the right to withdraw consent at any time;
9. Pursuant to the Article 12 of Regulation (EU) 2016/679 of the European Parliament and of
the Council of 27 April 2016 on the protection of natural persons with regard to the
processing of personal data and on the free movement of such data, in the event of a breach
of data protection legislation, you have the right to file a complaint to the supervisory
authority;
10. Your data will not be processed by automated means and will not be subject to profiling.
